Referrals

Who Can Refer?

Referrals are accepted from:

• General Practitioners (GPs)
• Medical Specialists
• Residential Aged Care Facilities
• Community Nursing Services
• Self-referrals are also welcome (with GP details provided)

To support a holistic and collaborative approach, we recommend that all referrals include the client’s nominating GP details. This allows our clinical team to liaise directly with the GP and ensure a coordinated care plan tailored to the client’s needs.

Once a completed referral form is received, our team will be in contact to arrange an appointment either face-to-face or via telehealth, depending on the client’s location and clinical requirements.
